How to use this template
Fill out the form before you start any work. Once you have touched the vehicle, any undocumented damage can become a dispute.
1 = Poor condition (heavy damage, staining), 5 = Excellent (minimal flaws). This gives you a quick reference for before/after comparisons.
Photos are your best protection. Take photos of any existing chips, scratches, dents, or stains before work begins. Store them with the inspection record. Mark Photos Taken = Y once done.
Ask the customer to sign the form before they leave. This confirms they reviewed and agreed to the documented condition. Keep a copy for your records.
Log any valuables or personal items left in the car (sunglasses, cash, car seats, documents). This protects you if a customer claims something is missing.
